Oral Rehabilitaion Using Implant in Oral Cancer Patients and Surgical Management in TMJ Ankylosis Patients

  As oral cancer surgery can result in distinct functional and cosmetic problems, reconstructive surgery is then recommended to improve patients’ quality of life.  The purpose of reconstruction of maxillofacial defects is to promote normal function as well as facial esthetics.  To achieve these results we have a large number of reconstructive options including primary repair with local tissue and the use of local pedicled or free flaps and/or bone augmentation technique. Our policy to the establish treatment goal in oral cancer patients is to achieve functional oral rehabilitation with implant-supported denture following anatomical reconstructive surgery. 
  On the other hand, combination treatment of bone augmentation including DOG (distraction osteogenesis) and implant are also applied to the patients suffering from TMJ ankylosis.  In such a situation, we perform oral rehabilitation using dental implants following gap formation of the TMJ using buccal fat pad grafting.
  The present paper describes our surgical strategy for functional oral rehabilitation using osseointegrated implants following many kinds of bone augmentation procedures in various oral cancer patients including TMJ ankylosis.
  In this paper, our most up-dated technique to distinguish oral epithelial dysplasia is also presented.

Trauma to the orbit is always complex, and adequate therapy requires that the surgeon be familiar with the detailed anatomy of the orbit and the pattern of injury of the soft and hard tissue components. Preoperative CT, MRI scans, or both are mandatory for diagnosis and proper planning of reconstruction.

Although generally-accepted guidelines have been developed for the maximal interval between trauma and reconstructive surgery, Controversies exist regarding the indications for surgery, the timing of surgery, and the best reconstruction material.


The management of orbital fractures has evolved with the advent of craniofacial techniques, computed tomography scanning, and rigid fixation. Accurate diagnosis followed by early and aggressive treatment of these injuries can limit the late complications often associated with orbital trauma. The fundamental principles in the management of facial fractures are adequate surgical exposure, anatomic reduction, rigid fixation, and reconstruction using autogenous bone. In this presentation, these key principles along with the pertinent anatomy, important technical points, and common pitfalls related to the management of orbital fractures will be discussed. Special considerations regarding management of post-traumatic complications will be also reviewed.
